FinalForms & Physical Info
For the 2025-26 school year, ACSC is continuing to use a completely paperless clearance process. The process includes a paperless IHSAA Pre-Participation Physical. A parent/guardian will be required to create an account and manage the necessary documents for all of their student/athletes using the FinalForms Portal. This process will provide a higher level of security and efficiency than the previous process. Step-by-step instructions to complete the process can be found below.
All information must be completed and submitted before any participation.
As a parent/guardian, you will register an account, then add your student to the account and complete only their information. Start creating your account using the link above. Then follow the steps below.
- From the landing page, click the Parent icon.
- Please register with your name as a parent, your email address, and create a password of your choice. When you are finished, click Register. (If you have multiple family members or if you have already registered yourself, you do not need to register again.)
